Insurance Information

Legacy Pain Associates is contracted with many health insurance plans, including Medicare, Cigna, US Family Health Plan and Tricare. You may want to check with your health insurance plan to find out if we are an in-network provider.

For your convenience, we will file the insurance claim for you. To expedite the paperwork, please make sure we have your most up-to-date insurance information.

Some insurance companies require additional information or require patients to see their primary care physician before we can see them. If you have any questions about your insurance, please feel free to contact our office prior to your appointment.

Cancellation Policy

Legacy Pain Associates values your time. When you schedule an appointment or procedure, we reserve a block of our physicians’ time just for you; therefore, if you fail to cancel your appointment or procedure there will be a fee. While we understand that circumstances may require rescheduling, we do ask that you give us 24-hours notice if you have to cancel.

Failing to cancel an appointment results in a $50 fee, and failing to cancel a procedure results in a $150 fee. Failing to cancel a New Patient appointment results in a $150 fee.
